I understand that it is my responsibility to determine that I am physically and medically able to participate in the Program and, if necessary, to consult with a physician prior to and regarding my participation in the Program. These seizures may have a variety of symptoms, including lightheadedness, altered vision, eye or face twitching, jerking or shaking of arms or legs, disorientation, confusion or momentary loss of awareness. Immediately stop participating in the Program and consult a doctor if you experience any seizure related symptoms.
